Disability Support Worker Job Description:
At Horizon Community Services, we are looking for passionate and dedicated Disability or Community Support Workers in Brisbane and Gold Coast to join our team of support stars. As a Disability or Community Support Worker, you will be responsible for supporting participants to enhance and maintain independent living skills and reach their potential at home and in the community in Brisbane, QLD.
Key responsibilities of the role include:
- Providing assistance with daily living activities such as personal care, meal preparation, and household tasks.
- Supporting participants to participate in community activities and social events.
- Providing transportation to appointments and activities.
- Developing and implementing support plans based on the needs and goals of the participants.
- Monitoring and documenting participant progress and reporting any concerns to the relevant parties.

The ideal candidate will possess the following skills and attributes:
- Previous experience in disability or community support work.
- Strong communication and interpersonal skills.

- Understanding and commitment to the principles of person-centered support.
Requirements for disability support worker in Brisbane:
- NDIS Worker Screening Clearance
- NDIS Worker Orientation module
- Police check (within the past 6 months)
- First Aid and CPR certificate
- Blue Card
- Driver’s Licence
- Evidence of relevant qualifications
- Evidence of Australian work rights (current visa or Passport)
